Tattoo Quote Form Template — Price by Size, Placement and Session Length
A session-length price plus size, placement, style and colour, with a deposit taken at quote time and a cover-up branch that requires photos before pricing.
Overview
A tattoo enquiry is a design conversation that needs a price anchor. The client has a reference image, a rough size and a body placement; what they do not have is any sense of how many hours the piece will take.
This template quotes by session length and adjusts for size, placement and technique. It also collects the reference images and the deposit in the same flow, because a design brief without a held date is a conversation that quietly expires.
Best for
Tattoo artists and studios that quote per session rather than per piece, and that need a deposit and a design brief before the calendar slot is committed.
The field list
Every question below earns its place: it changes the price, the visit or the crew. If a question does none of the three, leave it out.
| Field | Type | Why it is asked |
|---|---|---|
| Placement | Select (single) | Forearm, shoulder, ribs, hand or neck. Placement changes both the difficulty and the aftercare advice. |
| Approximate size (cm) | Number | The clearest proxy for session length, and the number the client is most likely to know. |
| Style | Select (single) | Fine line, traditional, realism or geometric. Each style has its own hourly rate and its own preparation. |
| Colour work? | Yes / No | Colour adds time and consumables, and it must be chosen at quote time, not in the chair. |
| Cover-up? | Yes / No | A cover-up is a different job with a different design brief and an assessment fee. |
| Session length | Select (single) | Two hours, four hours or a full day. The primary price driver and the unit the studio actually sells. |
| Reference images | File upload | A reference is the design brief. Without it the quote is a guess and the consultation starts from zero. |
| Skin condition | Text (optional) | Scarring, moles or a recent sunburn can change the plan. A short note is enough at quote time. |
| Age confirmation (18+) | Yes / No | A legal gate, not a preference. The quote must not continue without it. |
| Artist preference | Select (single) | Clients often choose the artist. Their rate and availability belong in the quote, not in a follow-up email. |
| Deposit tier | Select (single) | The deposit holds the date and is deducted from the total. Making it a choice keeps it visible. |
| Preferred days | Select (multiple) | Studio calendars are fragmented. Knowing the client's availability turns a quote into a booking faster. |
Price rules
Each row is one option in the builder with the operation attached to it. The example column is the arithmetic you should see in the live total; use it to check your own configuration before publishing.
| Option | Rule | Worked example |
|---|---|---|
| Minimum session (two hours) | +160 | 160 |
| Half day (four hours) | +320 | 320 |
| Full day (seven hours) | +560 | 560 |
| Fine line detail | +12% | 320 × 0.12 = 38.40 |
| Colour work | +15% | 320 × 0.15 = 48 |
| Realism shading | +20% | 320 × 0.20 = 64 |
| Cover-up assessment | +60 | 60 |
| Large piece (over 25 cm) | +25% | 320 × 0.25 = 80 |
| Hand or neck placement | +80 | 80 |
| Deposit (deducted from total) | +80 | 80, deducted from the final price |
| Design revision | +45 | 45 |
| Touch-up within six months | Included | No charge |
The amounts are illustrative, not a price list. Replace them with your own cost base and currency before publishing — and keep the order of the rules, because a percentage applied to the wrong subtotal changes every total downstream.
Conditional logic
Conditional rules keep the form short without losing the information. Add them in the order below and test each trigger before you publish.
If Age confirmation is No
Then Stop the form and show the studio's 18+ policy. No price, no deposit, no booking.
If Cover-up is Yes
Then Require the reference images and the cover-up assessment fee, and replace the instant total with "book an assessment". A cover-up cannot be priced from a size alone.
If Approximate size is under 10 cm
Then Hide the full-day session and default to the minimum session. Small pieces are not sold by the day.
If Placement is Hand or Neck
Then Show the placement surcharge and the visibility note before the total.
If Colour work is Yes and Style is Realism
Then Stack the colour and realism percentages and show both on separate lines, so the client can see why the total moved.
If Deposit tier is Standard
Then Show that the deposit is deducted from the total, not an extra fee.
Rebuild it in Clickobook
This is the build order we use. It ends with two test submissions, because a priced form is only correct when a total you calculated by hand matches the one on screen.
-
1
Create a "Your idea" section with placement, approximate size and style. Size and placement are the two answers that make the rest of the quote meaningful.
-
2
Add the session-length select with the two-hour, four-hour and full-day prices. Price sessions, not centimetres; a per-centimetre rate cannot survive a detailed small piece.
-
3
Add the technique modifiers — fine line, colour and realism — as percentages on the session price, then the flat add-ons: cover-up assessment, hand or neck placement and design revision.
-
4
Add the brief section: reference images, skin condition notes and the 18+ confirmation. Make the age question a hard gate.
-
5
Add the deposit as a price line with a clear "deducted from total" label, then test a small fine-line piece against a full-day realism cover-up.
-
6
Connect the deposit to your payment provider and the artist preference to your calendar, so the quote ends with a held date rather than an email thread.
Three things that make this template convert
Quote a range, not a single figure, for pieces that need a drawing first. Sessions can run long and a fixed price punishes the artist for it.
Take the deposit at quote time. A design brief without a deposit is advice given for free.
Show the included touch-up explicitly. It is a real cost that most clients assume is charged.
